DSISD athletic registration for student-athletes is submitted electronically through Aktivate (formerly Register My Athlete). If you need assistance, please click here for parent instructions or contact the DSISD Athletics Office at 512.858.3037.

The Physical Evaluation Form is required for all DSISD student-athletes, band, dance and cheer. The form must be completed annually by parent (or guardian) and student in order for the student to participate in activities.

NOTE: Do not download the student physical in Aktivate. This section will show as "Incomplete" until the athletic trainers verify receipt of the current year's physical.   

• High school athletes should turn in their completed UIL physical form to the athletic trainer in the Athletics Office.

• High school band, dance and cheer students may turn their forms in to their sponsor/director.

• Middle school athletes should turn in their physical forms to their coach before the first day of practice.
